When I clicked on "Show the Questionnaire with these Option", I then viewed a questionnaire with this URL:

It is this URL (which will appear in your browser's location) that you should copy and save. You can just send it to users, or hide it in a labeled link such as:

or it could be placed in the location/URL field in, say, Word.

You can use the Back button to get back to the original page and make changes and click on the "Show ..." button again. If you are adventurous, you could edsit the URL directly. Changing the email address is tricky, but it might be better than creating a questionnaire from scratch. On the other hand, changing the "form" paramneter from CSUQ to QUIS is not bad at all.

That's sad news about your mom. I hope your kids have fond memories of her.

Things are fine here.  This summer, and we had a steady stream of
visitors, so we were quite busy although we did not leave towm.  The
boys start school this week and we will fall back into a routine.

I don't know if the 83-94 HFES-HCI collection deserves a place in an HCI
history chapter, but I appreciate the thought, and I accept your
apology.  Last year, I tried to get someone to make a new collection for
95-06, but it does not seem to have had an effect.  I sometimes wonder
why bother maintaining the HCI Bibliography, especially during this year
in which I added close to 10,000 records.  A script for this site and
another for another, and the data from another pub is available for
searching on the same footing as the others. So that's the main reason:
to help people find HCI material they might have missed using google
or the ACM DL, or some other collection. After years of negelect because
no one cared that it was gone, I started adding HFES proceedings again,
in part because it was so easy to grab their data. Maybe with social
ratings and online access, the 83-94 HFES-HCI collection is just a
relic of pre-web thought, so maybe it IS a good thing to mention in
a history chapter.
