Dear Dr. Rammohan,

I hope you read email because getting through to 293-4964 seldom works.

As you may know, I bailed out of the MRI because I panicked in the tunnel.
I had no idea the test would be an hour and a half,
and I barely tolerated the last one, especially with the "shot".
I was rescheduled for July 9th,
and was told I'd be given "something" to calm me,
but I was still having nightmares and a lot of anxiety,
both about the tunnel and about the possibility of regular shots
(which I do not think I would tolerate well).
So I cancelled July 9th.

Are there any alternatives to the tunnel?
Are there any alternatives to such a long test
(could it be split up into two)?

Hi Mr. Perlman,

Sorry that you could not get through to my office. It is a nightmare and I
feel for all my patients. Hopefully we will have the infrastructure
problems worked out, but I have been hoping for that now, for some time.
They are truly terribly short-staffed.

The MRI can be done if you take a good tranquilizer like Xanax. I can call
that in before the MRI. The other possibility is to go into an open
machine. The pictures are not as good but if that is really the only option
it is still better than no pictures.

Don't start losing sleep over the daily injections. We will cross that
bridge when we get to it. Right now, let us see if we can get the MRI.
Please reschedule it, unless you feel that there is absolutely no way.

Sorry that it has been so diffficlut!

Rammohan

Dear Dr. Rammohan,

As you might know, I finally rescheduled the MRI for last week.
The Xanax worked very well, although I have little memory of
the MRI; I suppose that's part of the benefit of the drug.
I think that keeping my eyes closed and having music helped,
although maybe it was all the drug.

During the first week of September, my hands, which have been
a little numb for five years, felt a lot more numb, both feet
went numb up to my ankles, and I was numb around my anus to
my penis. Toward the end of September, most lost feeling had
returned (except my hands, which have reverted to their
normally tingly state), although my feet are still a little
tingly in the toes and perhaps (I'm not sure) in the heel.

Perhaps unrelated to MS but related to my back is that I've
had progressively worsening lower back pain that has not
responded to two months of Naproxen and was further irritated
by three weeks of physical therapy. That may be Dr. Kreger's
domain, and perhaps there is something in the MRI down that far.

Shall I schedule an apointment to follow up on the MRI?

Dear Dr. Kreger,

I'm having a hard time getting through to you on the phone.
I go through a series of voice menus and end up on hold
and then the system hangs up on me. I did finally get
through today after 4pm but you were not available.

I was on Naproxen twice a day for two months with little
change in my hand or back. I was not sure whether I could
take Ibuprofen with it, so I avoided it, but since I ran
out of Naproxen this week, I have been taking Ibuprofen
for pain.

I started physical therapy about a month ago and did
low level exercises for three weeks. My back got progressively
worse and last week I missed a day of work from severe low back pain.
The therapist ran out of exercises to get my back in good enough
shape for strengthening and we thought that perhaps I needed
more diagnosis or some way that I can do exercises without
debilitating pain. Using the therapists rating scale or
1 (no pain) to 10 (high), I started therapy at about a 6
and worsened during therapy to about an 8 and sometimes 9
(that's where I fall to my knees and wail). It's a dominating
factor in my life right now.

One thing that is clear from the therapy is that most of the pain
is on my left side.

I had a spine MRI last week (Tuesday 9 October) for Rammohan,
but I think it was thoracic and my problems are lower.
Nice spine shots here:
	http://www.cid.ch/DAVID/spin.html

My left thumb has not improved at all.

Dear Dr. Rammohan,

I have not been able to schedule a followup to my MRI
from last October 9. I think I had an appointment,
but your office cancelled it. When I tried again
last week, I was told that you are booked until September
(almost a year after the tests). I left my home and work
phone numbers for you to call with results. I would prefer
to be called at home so that my wife can ask questions.
Or, if I can be fit in for an appointment, she would
like to attend.

My work number is 761-5058 and our home number is 457-5508.
My SSN is 079-54-6667.

Gary Perlman
